| READ ALL INSTRUCTIONS BEFORE YOU START. | ||

| We would like you to collect both a swab that touches the opening of the penis and a urine sample. However, if you feel uncomfortable collecting the swab, you may collect just the urine (Step B). If you choose to collect both sample types, the swab (Step A) must be completed first. | ||
|
|
|
| The kit should contain (A) swab sample instructions and supplies, and (B) urine sample instructions and supplies. | ||

|
| Step A: Swab Kit Supplies and Sample Collection Instructions |

|
|
|
| Swab supplies: Swab and swab transport (SHIPPING) tube. Reminder: collect this swab before the urine. |
||
|
|
|
| If you are going to participate in the swab study (swabbing the area at the opening in the penis), begin with the following directions. | ||
|
|
|
| To collect the SWAB sample: See diagram. | ||
|
|
|

|
Open the swab package. |
|
|
|
|
|
Roll the swab just at the tip or inside the opening to the penis (through which you pee). Please be sure to roll the swab completely around the opening to get the best specimen. It is not necessary to put the swab deep inside the hole (urethra opening). |

|
|
|
Put swab into swab transport tube; break it off so it fits into tube, being careful not to spill the liquid. Tighten lid securely. |
|
|
|
|
|
Put the tube containing the swab into the plastic bag. |
|
|
|

| Urine supplies: Urine collection sponge stick and urine transport tube. Wait at least one hour from your last urination (peeing) to collect your sample. Be sure to collect urine after the swab. |

|
|
| To collect the URINE (pee) sample: | ||
|
|
|
|
|
Remove the urine sponge stick from the wrapper (don’t touch the sponge). |
|
|
|
|
|
Hold penis to direct stream (flow) of urine (pee) onto the sponge stick so that the sponge gets soaked with the first urine from the penis. |
|
|
|
|
|
Open urine transport tube making sure to keep the tube upright. |
|
|
|
|
|
Put the sponge stick (attached to the lid) into the urine transport tube. |
|
|
|
|
|
Tighten lid securely so that liquid will not leak. |
|

|
| PACKING & MAILING | ||
|
|
|
|
|
Put urine transport tube and swab transport tube (if done), into plastic bag and seal ziplock completely. |
|
|
|
|
|
Place plastic bag containing the transport tubes (one for urine, one for swab), contact sheet, questionnaire, and one signed consent form into pre-addressed, postage-paid mailer and drop into any mailbox. |

|
Thank you for participating. Don’t forget to call us in two weeks at the phone number on the contact form included in your kit for your results.
